For a) we tested and confirmed that the feature is working as expected.
The way that you add an extra fee on the PRO version is slightly different and way more powerful than the free version.
Steps- Add multiple fees, one per shipping method inside shipping zone, that you want to charge differently
- Restrict the fees using the pencil at the right of the fee row, choosing the respective shipping method
- Reorder fees in a way that the generic fee will be the last, and the shipping based fees at the top
